Alabama Church School Enrollment Forms
To enroll with the Alabama Church School, you must complete each of the forms below. Ensure that all relevant details for every school-aged child you wish to enroll are provided.
IMPORTANT: Before proceeding with enrollment, you MUST read our FAQ. All answers to your queries are detailed in the FAQ section. By enrolling, you acknowledge and confirm that you have read both the FAQ and our policy. Links to these are provided within the enrollment form for your convenience.
Online Enrollment Forms:
The initial enrollment fee is $99. Subsequent annual renewals cost $69.
Every form below needs to be filled out. There are no exceptions.
STEP 1: Enrollment Form (ENROLL IN ALABAMA CHURCH SCHOOL)
Enrollment Refund Policy: Please note our longstanding policy – once you enroll or re-enroll, the service is considered rendered. As such, there are no refunds upon withdrawal, regardless of the time of year. By signing the form, you confirm your understanding of this no-refund policy.
Attention: If you’re enrolling just ONE student, all you need to complete is the “STEP 1: Enrollment Form (ENROLL IN ALABAMA CHURCH SCHOOL)” as mentioned above. Use the additional Church School Enrollment Form only if you’re enrolling more than one student.
For Additional Students: If you’ve already completed the main enrollment form and need to enroll extra students, use the following:
STEP 2: Church School Enrollment Form
Note: Complete one form for each child. If enrolling just one student, this isn’t necessary as one student is already accounted for in the primary enrollment process. Submitting only this form without the main enrollment will result in deletion of your email without enrollment. This form is specifically for the Board of Education (BOE). Ensure that you’ve already enrolled in Alabama Church School before we can process this form.
For Returning Students: Re-enrollment Form
Note: This is strictly for re-enrollment. If you were enrolled the previous year, use this form. Ineligible submissions will be refunded without further explanation.

Update Family Information:
Family Information Form: Use this for initial contact details when enrolling and for any subsequent updates throughout the year. Alternatively, you can send us a detailed email with changes similar to the form’s content.
Academic Records:
Attendance Record: Maintain attendance logs, especially crucial for high school students.
Withdrawal Procedures:
Student Withdrawal Form (SWF): Complete this if you’re removing your child/ren from Alabama Church School.
Public School Withdrawal Form (PSWF): Exclusively for those transitioning from public schools. Note: Some institutions may have specific withdrawal protocols that you previously agreed to upon their enrollment.
Student Record Release:
Student Record Release Form (SRRF): For each student, print and sign. This is essential if you wish for us to request records from a prior school. If there’s no prior schooling, this isn’t mandatory for new enrollments unless specifically desired. Records will stay with us until you transfer and request them, bearing the shipping costs. They will not be randomly dispatched. All shipping costs are borne by the requester.

Use this page to request an annual transcript, particularly if your student is transferring to another institution.
High School Specific Requests:
Graduation/Senior Package Transcripts:
For students approaching graduation, use the Graduation/Senior Package Transcripts Request link to access the form. Ensure you submit it by December 15th of the year preceding graduation. After making this request, you can apply for your transcript post-April of the subsequent year. At this point, we’ll prepare the certified transcripts and dispatch both the transcripts and the diploma to your address.
Other Relevant Forms:
Driver’s License Enrollment/Exclusion Form (2022 Edition):
This form is necessary for acquiring a driver’s permit. Fill only the top section and forward it to ACS, ensuring you enclose a Self Addressed Stamped Envelope (SASE). Remember, post the administrator’s signature, you have a 10-day window to secure the permit. If unforeseen events prevent a DMV visit within this timeframe, resend the form with a new SASE. If you’ve scheduled the test date for your child, kindly attach a note mentioning it.
Note: Although some offices might provide a Home School Affidavit, remember, as per legal definitions, we’re categorized as a Church School.
Work Permits:
For students under 18 seeking employment, a work permit is essential. If your child is below 15 years of age, forward the form to Alabama Church School for an endorsement on the School Record segment.

Although we do not require a membership to the HSLDA it would be recommended to become a member. If you have any legalities, or questions about anything related to homeschooling, you can call them and get answers that are 100% accurate and helpful. We hold a membership with them and are very pleased. So, with that being said, I would recommend a membership, but it’s not a requirement.
