This Policy/Terms of Service may change at any given time, you should always re-read the Policy/Terms of Service before Signing!
I understand and have read the FAQ page completely and the Policy on this enrollment form or the Policy page.
I understand that it’s my responsibility to re-enroll my students each year without being contacted by anyone from ACS.
I understand that the email reminders to re-enroll that I receive from ACS are a COURTESY and I know my students need to be re-enrolled each year.
I do NOT hold ACS accountable or responsible if I fail to re-enroll my students.
I understand that it’s not the responsibility of ACS to call each parent because they haven’t re-enrolled.
As a homeschooling parent for many years, I NEVER needed a reminder. You don’t want to have truancy issues, so you re-enroll before August 15th EACH YEAR!
I agree it’s my responsibility to educate and teach my children at home.
I have read and agree to the following terms and failure to comply may result in dismissal.
I also acknowledge responsibility for choosing my child/student’s curriculum for each school year.
I will also keep and maintain a record of grades and work completed for each child.
I agree to contact the administrator if any of my contact information changes during the school year, or if you decide to withdraw from ACS.
I give the administrator authorization to report to our public school superintendent should our child(ren) cease enrollment with ACS.
I agree that I will submit grades to ACS so that an official high school transcript can be prepared and sent out in a timely fashion to the student.
I understand that I will not just be supplied with a transcript without the applicable fees, as well as submission of proper forms.
I release Alabama Church School from ALL responsibility and/or liability for the education, social and physical well-being of our child(ren).
I have read the guidelines and policies of ACS and understand/agree with them.
I agree and understand that the enrollment fee doesn’t cover children who are in another school but aren’t enrolling with ACS at this time.
I understand and read the ACS policy on student record-keeping and transcripts.
I understand that if a child moves in with another parent/family member, friend, enrollment does not transfer and a new enrollment is required
I do NOT hold ACS, the administrator, staff members, or volunteers liable for the education of my child, including purchasing of curriculum, grading, or testing.
I understand ACS is not state accredited.
I understand that ACS 100% places all of the responsibility of the education of my child on me, the parent.
I understand what to do if a truant officer or public school personnel ask about our enrollment or other educational decisions. (see FAQ)
I understand that I must re-enroll every year between June-August 15th using the re-enrollment form only!
I understand that it’s MY responsibility as a parent to re-enroll without being hounded, called, or continuously emailed.
I understand that it’s not the administrator’s place to call each person who is late enrolling, because it’s my responsibility as a parent to re-enroll without being reminded.
I agree and understand that I must use the Family Information Form if something changes in our current living situation, phone or email address changes. Update always using Family Information Form!
I understand that I am responsible for buying books, planning the curriculum, and keeping records of grades and work completed.
You understand that ACS reserves the right to dismiss a student at any time while enrolled if deemed necessary.
I understand that there is NO refund if I withdraw my child from the school at any time during the school year, as a service was performed upon enrolling or re-enrolling. There are NO refunds you agree that you understand when signing this form.
I understand there is NO refund in general at all! Once a check or payment has been made your child(ren) will be enrolled in ACS.
I understand that if I withdraw from ACS I will be required to request records for the new school by submitting grades and required fees!
The following are reasons for dismissal:
Not re-enrolling in a timely manner, by August 15th each year
Not paying bounced check fees or clearing them up in a timely manner
Failure to report any address or phone number changes
Any conduct that puts ACS in jeopardy or legal trouble
Parent or Child become involved in illegal activities
A child moving to another residence without informing ACS
Parent not having full legal custody or spouse doesn’t agree with home education
Disregard for policies ACS have set in place
Parents not being honest about any legal issues/custody issues/truancy issues
Not reading and complying with all policies and guidelines
Not keeping a copy of your CSEF file available at all times (if you need a copy, request it)
High School
This section is only required for High Schoolers:
I understand that if my child is a graduating student I am required to turn in the senior package fees by December 15th
I will submit an attendance sheet to ACS upon withdrawal or graduation.
I have read the information offered for the diplomas and understand what’s required for each one
I know a student must have ACT testing to apply to colleges and will apply for testing through the ACT testing website
I know certain subjects must be completed for Academic and College Prep Diplomas and take full responsibility for having my child complete them
