Thank you for being part of the Alabama Church School’s Graduating Students!!! This form is specifically for seniors who are graduating and need to request their official senior documents.
Please complete the form below with accurate and complete information. Our office will process your request and prepare the following items for you:
✔️ An official diploma
✔️ Two (2) sealed official transcripts for college use
✔️ Two (2) additional copies of the transcript for student records

Important Notes:

📚 Senior packages are prepared only for students who have fulfilled all graduation requirements.

📝 Be sure all final grades are submitted at the time of requesting your senior package.

⏳ Standard processing time is 14 business days from the date your completed request is received.

🚫 No refunds are issued for any transcript requests, diplomas, or senior package services once submitted.
⚡ Rush Processing Available:
If you require expedited service, a rush fee of $45 will apply.
Rush processing guarantees that your full senior package (diploma and transcripts) will be completed within 3 business days from the date your completed request and rush payment are received.
Please indicate on the form if rush service is needed and include the applicable payment.
