Online Re-Enrollment Forms
Each of these forms must be filled out in order to re-enroll in Alabama Church School. Please fill out all info and include any children that have reached school age this year. There is NO extra charge for adding a student. Each form will open in a new window.

The fee for Yearly Re-Enrollment is $69.

Additional Forms:
- Church School Enrollment Form (Optional, only if you move or you’re adding a student)
- Family Information Form (Optional, only if anything has changed)
Use the Additional forms for children reaching school age and are required to be enrolled in a church school, public school, or private school. You will also fill out a Family Information Form & Church School Enrollment Form if you add a student. If your address, phone, and email have changed, simply fill out a Family Information Form. If you’ve moved or your address changed and you’re in a new County, you’ll need both additional forms.
