Each of these forms must be filled out in order to enroll with Alabama Church School! Please fill each one out completely including all of the school-aged children you want to be enrolled.

- Church School Enrollment Form (One form per child is REQUIRED!)
Please complete the Church School Enrollment Form for EACH child being enrolled as per the B.O.E. Each child has their own form.

Each of these forms must be filled out in order to re-enroll in Alabama Church School. Please fill out all info and include any children that have reached school age this year. There is NO extra charge for adding a student. Each form will open in a new window.

- Church School Enrollment Form (Optional, only if your address and/or county has changed or adding a student.)
- Family Information Form (Optional, only if anything has changed after being enrolled.)
Use the Additional forms for children reaching school age and are required to be enrolled in a church school, public school, private school. You will also fill out a Family Information Form & Church School Enrollment Form if you’re adding a student. If your address, phone, and email have changed you simply fill out a Family Information Form. If your address changes and you’re in a new County, you’ll need both additional forms.
